If the worst pain of your day is the first few steps out of bed, and it eases once you get moving, that pattern is the signature of plantar fasciitis. It is the most common cause of heel pain, and summer hiking and running season is when it tends to show up.
What plantar fasciitis actually is
The plantar fascia is the thick band of tissue along the bottom of your foot. When it gets overloaded, it becomes irritated where it attaches at the heel. The pain feels sharp first thing in the morning because the tissue tightens overnight, then stretches abruptly when you stand. This is uncomfortable, but it is not a sign that your foot is damaged or that you have to stop the activities you love.
The treatments with the strongest evidence
The good news is that the treatments with the strongest evidence are the hands-on and active ones, not the gadgets. The 2023 clinical practice guideline from the Academy of Orthopaedic Physical Therapy, published in the Journal of Orthopaedic and Sports Physical Therapy, gives its highest grade of recommendation to manual therapy directed at the foot and ankle, plantar fascia and calf stretching, and orthoses used alongside other treatment. The same guideline recommends using these instead of electrotherapy machines like ultrasound. A 2025 evidence-based review in Cureus reached the same conclusion, placing stretching and loading exercise among the first-line therapies and reserving injections and surgery for the small number of cases that do not respond.
Why strengthening beats rest and stretching alone
Loading the tissue matters, not just stretching it. A randomized trial by Rathleff and colleagues, still a reference point in the field, found that people who did a simple high-load heel-raise exercise every other day reported better function at 3 months than those who only stretched. The strengthening did not replace stretching, it sped up the recovery.
Why we look at the whole leg, not just the heel
What this means for you is that heel pain is very treatable, and rest alone rarely fixes it. The source is often not only the heel. A stiff ankle, a tight calf, weak foot and hip muscles, or the way you load the foot when you run can all keep the fascia irritated, which is why we look at the whole chain rather than the sore spot.
