About this policy
This policy explains what information Enhance Physical Therapy collects through this website and through our advertising on Google, Facebook and Instagram, how we use it, and what choices you have.
This is not our HIPAA Notice of Privacy Practices. Protected health information created during your care as a patient is governed by a separate document that you receive at your first visit. This policy covers website visitors and people who contact us before becoming patients.
Information you give us
We collect information you choose to provide, including your name, email address, phone number, and anything you tell us in a contact form, a text message, an appointment request, or a form attached to one of our ads on Facebook or Instagram.
If you request an appointment, you are directed to our scheduling system, Jane App, which collects the information needed to book and manage that appointment. Jane App handles that information under its own privacy practices.
Information collected automatically
When you visit this site, certain information is collected automatically through cookies and similar technologies. This includes your IP address, browser and device type, pages you viewed, how long you stayed, and the website or advertisement that sent you here.
We use the following tools:
Google Analytics 4. We use Google Analytics to understand how people find and use this site. Google Analytics uses cookies to collect usage information. You can read about Google's practices at policies.google.com/technologies/partner-sites and you can opt out using the browser add-on at tools.google.com/dlpage/gaoptout.
Google Ads. We advertise on Google Search. Google uses cookies to measure which advertisements lead to appointment requests.
Meta Pixel. We use the Meta Pixel, a tool provided by Meta Platforms, Inc. Meta and other companies may use cookies, web beacons and similar technologies to collect information about your activity on this site and on other sites and apps, both to provide measurement services and to show you advertisements that may be relevant to you. You can manage how Meta uses your information for advertising in your Facebook settings under Ad Preferences, and at facebook.com/about/ads.
How we use your information
We use the information described above in order to respond to your questions and appointment requests, schedule and manage appointments, send you educational material or resources you requested, send occasional emails about our services if you have asked to receive them, measure which advertisements and channels bring people to our practice, improve this website, and meet our legal and professional obligations.
We do not sell your personal information.
Facebook and Instagram lead forms
If you complete a form attached to one of our advertisements on Facebook or Instagram, Meta collects the information you submit and passes it to us. That information typically includes your name, email address, phone number, and your answers to any questions in the form. We use it in order to send you the resource you requested and to follow up about whether we can help you.
Meta's own collection and use of your information is governed by Meta's data policy, which is available at facebook.com/privacy/policy.
Who we share information with
We share information only with service providers who help us operate the practice, and only to the extent needed for them to do that work. These currently include Jane App for scheduling and patient records, Stripe for payment processing, Go Getters CRM for managing inquiries and follow up communication, our email service provider, and the analytics and advertising platforms described above.
Each of these providers handles information under its own privacy practices. Stripe processes card payments directly and we do not store your full card number.
We may also disclose information where required by law, or where necessary to protect the safety of a person.
Your choices
You can unsubscribe from our emails at any time using the link at the bottom of any message.
You can ask us what information we hold about you, ask us to correct it, or ask us to delete it, by emailing doug@enhancept.net. We will respond within a reasonable period.
You can disable cookies through your browser settings. Some parts of this site may not work correctly if you do.
How long we keep information
We keep website and advertising inquiry information for as long as needed for the purpose it was collected, and generally no longer than three years after your last contact with us, unless a longer period is required by law. Patient records are retained separately according to Utah state requirements and professional standards.
Security
We use reasonable administrative and technical measures in order to protect the information we collect. No method of transmission over the internet is completely secure, and we cannot guarantee absolute security.
Children
This website is not directed to children under 13, and we do not knowingly collect personal information from children under 13. If you believe a child has provided us information, contact us at doug@enhancept.net and we will delete it.
Changes to this policy
We may update this policy from time to time. The effective date at the top of the page reflects the most recent version.
